研究課題名:『 fMRI・MRSを用いたヒト脳機能解明に向けた研究 』

要旨

 現在,核磁気共鳴計測は時間分解能が低いとされているが,われわれが提案する外部トリガーという手法を用いることで秒単位の分解能を持たせることが可能である.われわれはこれまで,この手法を適用した核磁気共鳴スペクトル法(MRS)を用いて,視覚刺激直後のヒトの脳内視覚野における神経活動としてコリンの減少をとらえた.一方,機能的核磁気共鳴画像法(fMRI)は脳内の神経活動をBOLD(blood oxygenation leveldependent)と呼ばれる信号としてとらえる.このBOLD の極めて初期に生じる一過性の信号減少と,われわれがとらえたコリンの減少を類似した現象として仮定し,代謝物質とBOLD の時系列変化を計測し比較した.その結果,被験者4人においてコリンがBOLD信号と類似した増減を示している.
 さらに,組織や器官の代謝物質を広範囲に測定することが可能なChemical ShiftImaging(CSI)という手法の計測条件について,MRI 画像をもとに計測ノイズを評価し,その可能性を検討した.
 また,視覚刺激に対する賦活に関して,視覚野の中心に正,周辺に負のBOLD(NegativeBOLD)信号が計測されることが報告されており,その要因には様々な説が挙げられ議論されているが,外部トリガーを適用したfMRIを用いて,ヒトの脳視覚野における視野調節を元に考えた試行を計測することで,この現象の解明の一考察を試みた.

はじめに

 現在,ヒトの脳機能測定は近年の技術革新により様々な手法を用いて研究されている.1980年代後半までは,ポジトロン断層法(positron emission tomography, PET)や単一光子放射断層法(single photon emission computed tomography, SPECT)を用いた測定が主流となっていたが,造影剤による放射性被爆という点で問題があった.一方で,核磁気共鳴(magnetic resonance, MR)を用いた手法は非侵襲的であり,とりわけ核磁気共鳴画像診断法(magnetic resonance imaging, MRI)はその空間分解能の高さゆえに,今日の画像診断において欠くことのできない手法の一つとして確立している.また,1990年代に入り,新たな脳機能の測定方法である機能的磁気共鳴画像法(functional magnetic resonance imaging, fMRI)が報告され,当初はMRIの撮像方法として用いられていた超高速撮像法であるエコープラナー法(Echo Planar Imaging, EPI)の機器に与える影響が大きかったために一般に普及しなかったが,近年の技術の向上により,EPI法での撮像が可能となり急速に普及した.そして,その他の脳機能測定法として,古くから行われている視覚誘発電位(visual evoked potential, VEP)に加え,脳磁図(magnetoencephalography, MEG)や近赤外分光法(near infrared spectroscopy)といった時間分解能の高い手法が開発され,様々な手法により脳機能の解明に向けた研究が行われている.
 核磁気共鳴スペクトル法(Magnetic Resonance Spectroscopy, MRS)はfMRIと同様にMRI装置で測定することができ,器官や組織における代謝物質の測定を可能にする核磁気共鳴を用いた手法で,現在では主に脳の腫瘍・損傷やアルツハイマー病などの中枢神経系の評価に用いられている.MRSはコイルを変えることにより様々な原子を対象に計測が可能であるが,中でも感度が最も高いH1-MRSが多く用いられている.代表的な計測対象の物質としてコリン(Cho),クレアチン(Cr),Nアセチルアスパラギン酸(NAA)が挙げられており,近年のMRSによる研究は,臨床的な疾患の評価だけでなく,健常者における代謝物質の変化をとらえたものも報告されている.
 このように,様々な脳機能測定手法がある中で,被験者に与える刺激とMR機器の撮像のタイミングを自由に操ることができる外部トリガー法という独自の手法を用い,時間依存のあるMR計測による脳機能測定を行うことで,健常者の脳機能のさらなる解明や臨床における新たな診断基準の設定,また,そのための計測条件・測定方法の検討が本研究の目的である.

 本研究は埼玉県所沢市にある国立障害者リハビリテーションセンター病院との共同研究であり,実験は医師・放射線技師の指導のもとに行い,被験者には説明と同意のもと計測を行った.

1.MRSを用いた代謝物質の時系列変化

 われわれはこれまで,外部トリガーを適用したMRSを用いて,視覚刺激直後のヒトの脳内視覚野における神経活動としてコリンの減少をとらえた(図2).この知見は外部トリガーの有効性を示すことはできたが,Choが減少する動態については解決していない.
 一方,fMRIが計測する脳内の神経活動はBOLD(blood oxygenation level dependent)信号として計測されるが,このBOLDの極めて初期に生じる一過性の信号減少(Initial-dip)と,われわれがとらえたコリンの減少を類似した現象として仮定し,代謝物質とBOLDの時系列変化を計測し比較した.
 視覚刺激1 秒に対する視覚野のBOLD 信号を計測した際に用いたfMRI シーケンスの内容をMRS 専用にPhase1・2・3における計測を行うシーケンスに改良し,VOI は20×20×20mm(= 8000 mm3),収集回数99 回で1人約2回ずつ計測.計4人において計測を行った.
 Tukey-Kramer's HSD test後,Dunnett's multiple comparison test を行った結果,CrとNAAに関しては3試行すべてにおいて有意差は見られず,Choについても閉眼安静時と連続刺激において有意差が見られなかったが,外部トリガー法を用いた刺激は他の試行に対してChoの約20%の減少がみられた(P < 0.01).
 現在の状態でCho においてBOLD との相関性が見られているが(P < 0.017).分散が大きい.今後は人数を増やしサンプル数を集めていくか,被験者の負担を考慮しつつ一度に計測する収集回数を増やすことを行っていく.

2.Chemical Shift Imaging の計測条件の検討

 MRSには単一のボクセル内の代謝物質を計測するSVS(Single Voxel Spectroscopy)の他に,CSI(Chemical Shift Imaging)という一度の計測で広範囲なボクセルデータを得る手法がある.この手法は一般的に正常組織と病理組織のスペクトル比較に用いられている.CSIは,広い範囲に渡り静磁場を均一にして計測する必要があることや,計測における位相エンコーディングにより隣接ボクセルの信号が影響を受けるため,広範囲な計測ができる一方でSVSよりも精度が低い.また,一度の計測に大量のデータを扱うため,計測時間や後処理の所要時間が長くなるなどの欠点がある.
 現在われわれが使用しているMR装置は,SVSにのみ外部トリガーを適用することができCSIには適用できない.しかし,CSIは広範囲な代謝物質の時系列変化を計測する可能性を秘めている.そこで,どのような計測条件が精度よく計測できるかを検討した.

計測条件の評価

 CSI は,広範囲のVOI による磁場の均一性や不完全な水・脂肪抑制によるアーチファクトの影響を受けやすいため,SVS に比べS/N 比(Signal to noise ratios)が低い.われわれはSVS と同様の計測条件(head コイルの受信設定であるManual receiveradjustment を行い,各計測直前におけるAuto Inline adjustment は行わない)でCSI 計測を行ってみたが,ノイズが多く毎回計測するデータがばらばらであった.そこで,同一被験者において以下の3つの計測パターンでデータを収集し,CSI の計測条件の評価を行った.

  A: Manual receiver adjustment あり,Inline adjustment なし
  B: Manual receiver adjustment なし,Inline adjustment あり
  C: Manual receiver adjustment あり,Inline adjustment あり

 関心領域(Volume of Interest:VOI)は,脳室周辺の信号強度が上がらないことに注目し図6のように設定した.シーケンスはCSI-SE135,TR=1500ms,1計測にかかる時間は約6分30秒で,各計測パターンを3回ずつ計測し,平均の分布を比較対象とした.
 図7はその結果であるが,軸位断のリファレンスイメージ(T1 強調画像)における脳室の形状が代謝物質の分布に形状として現れているのが視覚的にわかる.そこで,脳室の形状を客観的に評価するため,軸位断のT1強調画像のピクセルデータを白黒の濃淡で数値化し,それを元に各代謝物質の分布との相関の度合いを比較した.さらに,VOI 全体をArea1,VOI にける脳室の左右の実質を除いた領域をArea2,脳室周辺のみをArea3 として,それぞれの相関・順位相関を行った結果が表1・2である.
 相関・順位相関ともに,B(Manual receiver adjustment なし,Inline adjustment あり)の計測方法が最も精度よく計測できていると評価でき,SVS 計測とは異なる計測条件が必要であるという結論に至った.表2におけるArea3でInline Adjustment を行わない方が相関係数が高い結果となっているのは,差が少ない結果を順位相関の順位決めにより強調されたために生じたものであり,Inline adjustment を行う方が適切であると結論づけることができる.また,Area によって結果が異ならないことから,計測による脳実質におけるノイズの評価も同等に考えてよいことが示唆された.
 これらの結果から,CSI はSVS に比べ広範囲を計測するため,Manual receiveradjustment によるADC の値を絞ることができないためManual による調節を行わない方がよいと考えられ,また,1度の計測による磁場の安定度を維持するために,毎回の計測時にInline adjustment を行う方がよいということが考えられた. さらに,この評価により決定した計測条件下で視覚野の計測を試みたところ,VOI が脳実質から外れた場合,極端に強い信号が検出され,周囲のボクセルに影響を及ぼしていた(図8).この結果より,皮質側を計測する場合はVOI が外部領域を含まないように設定する必要があることが分かった.

3.ヒトの脳視覚野におけるNegativeBOLD信号と視野調節

Negative BOLD Responses and Central-Surround Modulation of Visual Field in Human Early Visual Cortex

Abstract
How should we understand negative BOLD responses (NBR) in fMRI studies? The causes have been controversial and the various factors have been reported such as neuronal inhibition, event related, blood stealing, circulatory condition and baseline problem. Therefore, we measured the dynamic responses in the human visual cortex to visual stimuli based on the center-surround modulation using fMRI with external trigger method which enables the accurate and precise measurements. After we confirmed the relation between visual field (VF) and classical receptive field (CRF) in the early visual areas as preliminary experiments, performed measurements with foveal-peripheral VF stimuli and represented the spatial-temporal mappings by time dependent analysis. Moreover, we measured the BOLD responses curves in the early visual areas to different stimulus durations, and discussed the relation between NBR and the surround suppression. As a result, we demonstrated that the causes of negative BOLD signals can be distinguished as a factor of neural suppression and the circulatory condition by measuring the time series changes. Furthermore, our findings support the importance of time series measurements in fMRI studies.
